Job Summary

Aptive Resources is seeking a dedicated and compassionate Social Worker to provide comprehensive social work services to veterans and their families within the Veterans Affairs Northern California Health Care System. The role focuses on Mental Health, Community Care, Medical Social Work, and Homeless Services. The Social Worker will deliver services across inpatient, outpatient, and outreach settings, emphasizing direct care.

 

This position will be supporting multiple facilities within the VA Northern California Health Care System.

Primary Responsibilities

Direct Care Provision:

  • Provide individual, family, and group psychotherapy for clients with serious medical and mental health issues.
  • Develop and implement appropriate treatment plans.
  • Conduct initial and ongoing assessments to determine service needs and referrals.

Clinical Services:

  • Offer clinical services including assessment, referral, case management, and psychoeducation for individuals and groups.
  • Employ therapeutic interventions based on training and privilege level.
  • Establish and maintain therapeutic relationships to assist clients in addressing their issues.

Case Management:

  • Evaluate client progress towards goals and provide follow-up after referrals.
  • Adjust treatment plans as necessary if results are inadequate.
  • Monitor client progress and maintain comprehensive case documentation.

Collaboration and Compliance:

  • Explain client treatment and progress to third parties with jurisdiction.
  • Ensure work is compliant with the National Association of Social Worker’s (NASW) Code of Ethics, Social Work Service Performance Standards, and all relevant laws and regulations.

Knowledge and Skills Application:

  • Apply principles and theoretical concepts of Mental Health social work.
  • Utilize administrative mechanisms appropriate to the assignment.
  • Understand program objectives of State and Federal agencies to ensure appropriate action.

Minimum Qualifications

  • Education: Master’s Degree in Social Work (MSW) from a CSWE-accredited graduate program.
  • Experience: Minimum of 1 year experience providing mental health services, preferably in a health-related setting.
  • Licensure: Valid State Licensed Clinical Social Worker (LCSW) License or State equivalent required for Mental Health positions.
  • Competence in general social work functions.
  • Ability to develop appropriate treatment plans and make informed service decisions.
  • Strong case management and documentation skills.
  • Proficiency in employing therapeutic methods and maintaining therapeutic relationships.
