Job Summary

Job Title: Registered Nurse (RN) – Community Care & Referral Coordination
Location:

  • Office of Community Care (OCC), 4100 E. Mississippi Ave, Glendale, CO 80246
  • Rocky Mountain Regional VA Medical Center (RMRVAMC), 1700 N. Wheeling Street, Aurora, CO 80045

Employment Type: Full-Time, On-Site
Hours: Monday–Friday, 7:30 AM – 5:30 PM (excluding federal holidays)

Pay: $40.00/hr plus 4.93 for Health and Wellness. Position will also provide benefits, PTO, and sick leave.

Position Overview:

We are seeking dedicated and experienced Registered Nurses (RNs) to provide professional nursing services to Veterans as part of the Office of Community Care (OCC) and Referral Coordination Team (RCT). This position focuses on coordinating high-quality care and ensuring smooth transitions between VA and community-based services.

Primary Responsibilities

Key Responsibilities:

OCC Nurses:

  • Perform clinical reviews of consults/requests for completeness and appropriateness.
  • Coordinate care and ensure documentation is thorough and timely.
  • Communicate with veterans, community providers, internal staff, and administrative teams to facilitate and process care authorizations.
  • Document communication and care coordination efforts in the electronic health record (EHR).

RCT Nurses:

  • Triage specialty consults using National RCI Standards for coordinated care.
  • Conduct veteran outreach to discuss specialty care options and eligibility for Community Care.
  • Serve as a clinical resource to multidisciplinary teams and liaise with both VA and community providers.
  • Support development and improvement of consult templates and workflows.

Minimum Qualifications

Qualifications:

  • Graduate of an accredited nursing program (ACEN or CCNE).
  • Current and unrestricted RN license in any U.S. state or territory.
  • Minimum of 2 years direct patient care experience within the last 3 years.
  • Current Basic Life Support (BLS) certification.
  • Proficient in written and verbal English.
  • Must be able to pass background checks, credentialing, and health screenings as required by the VA.

Preferred Skills:

  • Experience in care coordination, case management, or utilization review.
  • Familiarity with VA systems, EHRs, and government healthcare procedures.
  • Strong communication and interpersonal skills for veteran engagement and interdisciplinary collaboration.

Additional Requirements:

  • Must complete mandatory VA training (e.g., HIPAA, Patient Safety, Infection Control).
  • Must meet VA immunization requirements (TB test, Flu, MMR, etc.).
  • Participation in team huddles, staff meetings, and continuing education is required.
